Submission of an online appointment form constitutes a request for an appointment only and does not create a dentist-patient relationship or guarantee an appointment. Appointment requests are subject to review, scheduling availability, and approval by BALTAZAR DENTAL.

• Appointments are only confirmed after clinic staff review and approval.
• Requested dates and times are subject to dentist availability.
• The clinic may contact you to modify or adjust scheduling.

You are responsible for ensuring that all information submitted, including your full name, contact details, email address, and any appointment notes, is accurate, complete, and up to date.

The clinic is not responsible for delays, failed communications, or missed appointments resulting from inaccurate information.

An appointment is considered confirmed only after you receive an official confirmation from BALTAZAR DENTAL through an authorized communication channel (SMS, telephone call, or email).

• Confirmation is issued by clinic staff, not by the website.
• Confirmation is delivered through SMS, a telephone call, or email.

An automatically generated acknowledgment that your request has been received does not constitute appointment confirmation.

Patients should notify the clinic as soon as possible if:

• They cannot attend the appointment
• They need to reschedule
• Their information changes

When cancelling or rescheduling, please observe the following:

• Notify the clinic at least 24 hours in advance whenever possible.
• Repeated missed appointments may affect future scheduling priority.
• Emergency cancellations are exempt from the notice period.

The clinic reserves the right to:

• Reschedule appointments when necessary
• Cancel appointments due to availability, emergencies, or operational reasons

Patients arriving more than 15 minutes late may have their appointment shortened, rescheduled, or cancelled depending on the remaining schedule and the discretion of the attending dentist. Late arrival may also result in:

• Reduced appointment time
• Rescheduling to a later slot
• Additional waiting time

The clinic will make reasonable efforts to accommodate patients whenever possible.

The online appointment system is intended solely for scheduling appointments. Information submitted through the website is not monitored continuously and must not be used for medical emergencies. It is not:

• A medical consultation
• A diagnosis platform
• Emergency medical advice

If you experience severe pain, uncontrolled bleeding, facial swelling, trauma, or any other urgent dental condition, contact the clinic immediately or seek emergency medical care.

The online appointment system must not be used for emergency dental care.

In a dental or medical emergency, contact the clinic directly by phone or proceed to the nearest emergency facility.

Patients agree to:

• Arrive on time for confirmed appointments
• Follow post-treatment instructions
• Inform the dentist of any allergies
• Inform the dentist of any medications being taken
• Inform the dentist if pregnant
• Provide truthful information and respect clinic staff

The attending dentist reserves the right to modify, postpone, or decline treatment when clinically necessary or when additional examination is required.

Patients under eighteen (18) years of age must be accompanied by a parent or legal guardian unless otherwise permitted by applicable law.

BALTAZAR DENTAL shall not be liable for delays, cancellations, interruptions, data transmission failures, internet outages, or other circumstances beyond its reasonable control.

• Missed appointments caused by incorrect patient information
• Technical failures outside reasonable control
• Delays caused by communication problems

While reasonable efforts are made to maintain the accuracy and availability of the online appointment system, uninterrupted access cannot be guaranteed.

Users agree not to engage in prohibited activities, including but not limited to:

• Automated bookings or the use of bots
• Spam or submitting false requests
• Uploading malicious files or code
• Impersonating another person
• Attempting to access other patients' information
• Attempting unauthorized access or disrupting website operation

The clinic may restrict access from abusive users.

All website content, including logos, graphics, text, and design, remains the property of BALTAZAR DENTAL and may not be reproduced without written permission.

In the event of any dispute relating to these Terms or the online appointment system, both parties agree to first attempt to resolve the matter through good-faith communication before pursuing any available legal remedies.
